Chris Chodan from Alberta Human Resources and Employment explains why so many people get hurt.

"Most common problems I would think is people not assesing hazzards is what it boils down to. If you look at almost every investigation that our officers do into a fatality or serious injury there is always several things that weren't done or were done that shouldn't have been and in retrospect or hind sight / 20-20 it's always really obvious and in fact would be most of the time if everyone involved would have sat down to look at it ahead of time."
